Energy isolation is unforgiving because the failure mode is sudden and severe. The Factories Act 1948 sets duties around the safe operation and maintenance of machinery, BIS IS 15652 informs isolation practice, and every serious site backs these with its own LOTO standard operating procedure. The classic incident is not ignorance of the rules but a shortcut under production pressure, isolating the main supply while forgetting a pneumatic accumulator, or assuming a colleague verified zero energy. The trainee is assigned a maintenance task on a virtual machine with multiple energy sources. They start by notifying affected staff that the equipment is going down, then move to shut down and isolate, working through the electrical supply, a pneumatic line and a stored-energy accumulator rather than stopping at the main switch. They apply their personal lock and tag to each isolation point, building the one-person-one-lock discipline. Crucially they then verify the zero-energy state by attempting to start the machine and confirming no motion or stored release; skip this and the simulation demonstrates the residual energy they failed to bleed. With work notionally complete and the area confirmed clear of people, they remove their locks in order and authorise a controlled restart.
Scoring & certification
Every attempt is scored across the sequence: affected staff notified, all energy sources shut down and isolated, locks and tags applied, zero-energy state verified, and a controlled restart authorised. The decisive failures are captured explicitly, an unisolated secondary source, a missing lock, and above all a skipped verification, so an assessor sees whether the trainee actually proved zero energy or merely assumed it. Per-step weighting yields an overall competency outcome and a passing run issues a dated certificate against the worker's record. Results flow over xAPI and SCORM to the LMS and the DrillXR compliance dashboard, where a maintenance manager can confirm only verified-competent staff are authorised to apply isolation and can evidence that competence to an auditor.
Deployment on your site
Lockout / Tagout runs on Meta Quest, Pico and PC-VR and launches in kiosk mode, so a headset in the maintenance bay boots straight into the module for the next technician. The scenario is configurable to the customer's plant: the specific energy sources on a machine, the number and type of isolation points, the lock-and-tag standard and the site LOTO SOP can be mirrored so the training matches the equipment crews actually service.
